RESUMO

We present two ICU-hospitalized patients with coronavirus disease-19 (COVID-19) presenting with endogenous endophthalmitis in one eye and variable manifestations of chorioretinitis in the fellow eye. Two diabetic patients (57 and 62 years old) showed anterior uveitis and yellowish-white subretinal infiltrations. The fellow eye of one patient showed patches of choroiditis, while the other showed full retinal thickness infiltrations. A workup yielded high serum titers of galactomannan, diagnostic of aspergillosis. The widespread use of high doses of corticosteroids in the management of COVID-19 may predispose to various secondary fungal opportunistic infections and may manifest in different forms of chorioretinal infiltration.

The current investigation was carried out to estimate the protective effect of aqueous extract of Cheatomorpha gracilis (AEC) against High fat Diet (HFD) induced liver damage in mice. The results of the in vitro study showed that AEC have higher antioxidant capacities in the DPPH and hydroxyl radical-scavenging assays. Indeed, many phenolic compounds (gallic acid, quercetin, naringenin, apigenin, kaempferol and rutin) were identified in the AEC. In the animal studies, during 6 weeks, HFD promoted oxidative stress with a rise level of malonaldehyde (MDA), protein carbonyls (PCOs) levels and a significant decrease of the antioxidant enzyme activities such as superoxide dismutase, catalase and glutathione peroxidase. Interestingly, the treatment with AEC (250 mg/kg body weight) significantly reduced the effects of HFD disorders on some plasmatic liver biomarkers (AST, ALT and ALP) in addition to, plasmatic proteins inflammatory biomarkers (α2 and β1 decreases / β2 and γ globulins increases). It can be suggest that supplementation of MECG displays high potential to quench free radicals and attenuates high fat diet promoted liver oxidative stress and related disturbances.


A presente investigação foi realizada para estimar o efeito protetor do extrato aquoso de Cheatomorpha gracilis (AEC) contra o dano hepático induzido por dieta rica em gordura (HFD) em camundongos. Os resultados do estudo in vitro mostraram que os AEC têm maiores capacidades antioxidantes nos ensaios DPPH e de eliminação de radicais hidroxila. De fato, muitos compostos fenólicos (ácido gálico, quercetina, naringenina, apigenina, kaempferol e rutina) foram identificados no AEC. Nos estudos em animais, durante 6 semanas, HFD promoveu estresse oxidativo com aumento do nível de malonaldeído (MDA), níveis de proteína carbonil (PCOs) e diminuição significativa das atividades de enzimas antioxidantes como superóxido dismutase, catalase e glutationa peroxidase. Curiosamente, o tratamento com AEC (250 mg / kg de peso corporal) reduziu significativamente os efeitos dos distúrbios de HFD em alguns biomarcadores hepáticos plasmáticos (AST, ALT e ALP), além de biomarcadores inflamatórios de proteínas plasmáticas (reduções α2 e β1 / β2 e γ aumenta as globulinas). Pode-se sugerir que a suplementação de MECG apresenta alto potencial para extinguir os radicais livres e atenua o estresse oxidativo do fígado promovido pela dieta rica em gordura e distúrbios relacionados.

RESUMO

Simple, accurate, precise, and rapid extractive spectrophotometric method was developed for the determination of four antipsychotics drugs, namely sulpiride (SUP), olanzapine (OLP), clozapine (CLP) and aripiprazole (ARP) both in tablets and in biological fluids. The method was based on the formation of red colored ion-pair complex between the studied drugs and eriochrome black T (EBT) with absorption maxima at 514 nm. The stoichiometry of the complexes in either case was found to be 1: 1 and the conditional stability constant (Kf) of the complexes have been calculated. Reaction conditions were optimized to obtain the maximum color intensity. Beer’s law was obeyed in the concentration ranges of 4-30, 4-20, 2-18 and 4-26 μg/ml with SUP, OLP, CLP and ARP, respectively. Various analytical parameters have been evaluated and the results have been validated by satistical data. The proposed method was successfully applied to the analysis of commercial tablets containing the drugs and the results were in good agreement with those obtained with reported methods. The proposed method was further applied to the determination of the studied drugs in spiked human serum and urine. A proposal for the reaction pathway was postulated.

Mammalian renal development differs from that of most of the other organs in that it proceeds through a series of three successive phases, each is marked by the formation of a more complex pair of kidneys. These kidneys are called the pronephros, mesonephros, and metanephros. The current study has been conducted to elucidate the developmental changes of rabbit kidneys during their pre- and postnatal life. In the present study we used samples, at different ages, of the rabbit kidneys from embryonic day 15 [E15] till maturity for light and scanning electron microscopical investigation. At E15, large mesonephros, occupied a great part of abdominal cavity of rabbit embryo, and undifferentiated metanephros were noticed. At E19 metanephros became differentiated into cortex and medulla with the initiation of nephron-forming stage. Additionally at this stage, the caudal series of mesonephros still detected but in degenerated and atrophied structure. Just before birth, at E30, the kidney demonstrated well-developed renal corpuscles, differentiated proximal and distal convoluted tubules, with apparently detectable loop of Henle and large collecting ducts. Shortly after birth rabbit kidney showed further morphogenesis and at two-months old, the kidney of rabbit with mature histological structure of the renal parenchyma was documented. In conclusion, the development of rabbit kidneys occurred mainly during the prenatal period while their histological maturity occurred from two to three weeks after birth. The present findings were discussed with previous publications

To address the question if an altered oral microbiota is associated with atherosclerosis. Twenty patients suffering from atherosclerosis and 10 controls were recruited. Clinical oral, medical and laboratory investigations were performed. Oral bacteria were collected and 16S rDNA was sequenced following Single strand conformation polymorphism.(SSCP) Probing pocket depths in patients were significantly elevated. The oral microbiota of patients and controls were dominated by Fusobacterium (16%/17%), Streptococcus (21%/14%), Prevotella (10%/12%), Enterococcus (12%/12%), Porphyromonas (8%/7%), TM7 (0%/7%) and Veillonella (6%/7%). Differences in diversity were not significant between groups. The pathology of atherosclerosis may not be related to significant qualitative changes of the oral microbiota.

To assess subclinical atherosclerosis in subtypes of hypertension using lipid profile as a biomarker and B mode ultrasonography of the carotid arteries. Ninety-six subjects [49 females and 47 males] aged 42-78 years were recruited from the vascular Doppler unit at Baghdad Teaching Hospital, Baghdad, Iraq from January to June 2008. They were grouped into normotensive [group I], isolated systolic hypertension [group II], isolated diastolic hypertension [group III], and combined systolic and diastolic hypertension [group IV]. Subclinical atherosclerosis was observed in groups III and IV in terms of significantly low levels of high-density lipoprotein, high levels of low-density lipoprotein, and high atherogenic index. The mean intima media thickness was significantly increased with transition from group I to group IV in all carotid arteries, and the mean resistive index value of each carotid artery in group IV was significantly higher than that of group I. Carotid plaque was demonstrated in a significantly higher percent in group IV [12 out of 15] followed by groups III [14 out of 31] and II [1 out of 30]. Lipid profile and high resolution B-mode ultrasonography of the carotids are good predictive measures of subclinical atherosclerosis. Isolated diastolic hypertension and combined systolic-diastolic hypertension accounts for early subclinical atherosclerosis compared with isolated systolic hypertension

To demonstrate the immunohistochemical and epidemiological characteristics of gastrointestinal stromal tumors [GIST] in a Middle Eastern population. This is a retrospective analysis of all intra-abdominal mesenchymal tumors [excluding childhood embryonal rhabdomyosarcoma and small round blue cell tumors] collected from the archives of the Pathology Departments of King Hussein Cancer Center, Amman, and King Abdullah University Hospital, Irbid, Jordan between 2001 and 2008. The immunohistochemical profile of all cases was studied at King Hussein Cancer Center, Amman, Jordan, between January and August 2009. Gastrointestinal stromal tumors comprised 45% of the intra-abdominal mesenchymal tumors [42 out of 93 cases], with the most common site being the stomach [n=17, 40.5%]. Twenty-seven GIST cases [64.3%] were classified as high risk, 4 [9.5%] as intermediate risk, 6 [14.3%] as low risk, and 2 [4.8%] as very low risk. Immunohistochemistry showed diffuse and strong positivity [+3] for CD117 in 85.7% of GIST cases, and for CD34 in 65% of cases. The high-risk tumors were more common in male patients [M:F=1.7:1], while the non-high risk tumors were more common in female patients. The immunohistochemical profile of GIST in Jordanian patients is similar to previously published data from other populations, with a slight male preponderance for high-risk GISTs

RESUMO

Non-steroidal anti-inflammatory drugs [NSAIDs] are associated with the development of diaphragm disease. We report a 73-year-old male patient with this condition who had used NSAIDs for 2 years. He presented with general weakness, syncopal attacks, and a short history of melena. At laparotomy, multiple areas of constricted bowel were found in the resected proximal jejunum. Gross and microscopic examination confirmed diaphragm disease. The relevant literature is reviewed

RESUMO

This study provided data by a simple method of acquiring information related to ambulance response time (ART) and determined whether it met the international standards of ART. Additionally, this paper also compared the duration of ART at this hospital before and after the implementation of an Emergency Medical Dispatcher (EMD) program. The ART, which started when details like phone number of the caller, exact location of the incident and the nature of the main complaint were received and ended when the emergency team arrived at the scene of the incident. The parameters recorded include call processing time, time taken to prepare the team and time taken to travel to the scene. The results of the study revealed that the ART for the university hospital (HUSM) was at 913.2 +/- 276.5 seconds (mean +/- SD) and it was far below the international standard of ART as a benchmark of a good ambulance service. However, the study suggested that the EMD program that was recently implemented at the HUSM gave a significantly improvement to the ART score.

This cross-sectional study was designed to determine the socio-demographic characteristics and prevalence of overweight/obesity and self-reported diabetes mellitus, hypertension and heart disease among the population in Pulau Kundur, Kota Bharu, Kelantan, Malaysia. This study was conducted in September 2005. We randomly selected 120 of 240 households in Pulau Kundur, Kota Bharu, Kelantan. Fifteen interviewers were trained to use a structured questionnaire to interview 348 adult respondents age 18 years and older in the selected houses. The mean age was 40.7 years; 52.7% were females and 99.4% were Malay. Sixty-two point seven percent were married and 50.9% of them had Ujian Penilaian Sekolah Rendah (UPSR) or less education. The mean head of family income was RM 532.4. The mean body mass index was 25.3. The overall prevalence of overweight/obesity, known hypertension, diabetes mellitus and heart disease were 49.1, 12.6, 7.8 and 2.0% respectively. Adults in this village had a high prevalence of overweight and obesity and self-reported chronic diseases. Health education and lifestyle modification are needed for those adults.

This crossover study assessed the effectiveness of plain water (PW), sports drink (SD), fresh young coconut water (CW) and sodium-enriched fresh young coconut water (SCW) on whole body rehydration (R) and plasma volume (PV) restoration after exercise-induced dehydration. Ten healthy male subjects ran at 65% of VO2max in an environmental temperature of 32.06 +/- 0.02 degree C with a relative humidity (rh) of 53.32 +/- 0.17% for 90 minutes to lose 3% body weight (BW). During the 2-hour rehydration period, subjects drank, in randomized order, PW, SD, CW or SCW equivalent to 120% of BW lost in three boluses representing 50, 40 and 30% of the fluid lost at 0, 30, and 60 minutes, respectively. In all trials subjects were still somewhat dehydrated even after the 2-hour rehydration period. Indexes of percent rehydration with PW, SD, CW and SCW were 58 +/- 2, 68 +/- 2, 65+/- 2 and 69 +/- 1%, respectively, with significantly better rehydration with SD and SCW. The rehydration indexes for SD and SCW were significantly lower than PW (p < 0.01). PV was restored to euhydration levels after 2 hours of rehydration with SD, CW and SCW but not with PW. The plasma glucose concentration were significantly higher when SD, CW and SCW were ingested. SCW was similar in sweetness to CW and SD but caused less nausea and stomach upset compared to SD and PW. In conclusion, ingesting SCW was as good as ingesting a commercial sports drink for whole body rehydration after exercise-induced dehydration but with better fluid tolerance.

The concentration of plasma sialic acid was estimated using the modified chemical method and the more sensitive enzymatic method in 20 subjects with impaired glucose tolerance and 20 control subjects. The mean sialic acid concentration values of the control subjects and subjects with impaired glucose tolerance using the enzymatic method were 1.747 +/- 0.047 and 2.583 +/- 0.070 mmole/l and 1.753 +/- 0.067 and 2.591 +/- 1.02 mmole/l for the chemical method. The intra-assay coefficient of variation for the control subjects and for the subjects with impaired glucose tolerance were 1.963% and 1.583%, respectively, for the enzymatic assay and 2.728% and 2.431%, respectively, for the chemical assay. The inter-assay coefficient of variation for the control subjects and for the subjects with impaired glucose tolerance were 2.686% and 2.723% for the enzymatic assay, and 3.819% and 3.95% for the chemical assay. Since the values do not differ significantly, the chemical assay is a cost effective method that can be used in large epidemiological studies.

This study was carried out to evaluate the endogenous postmortem [PM] ethanol production in Albino rabbit. Fifty-four adult male new-zeland rabbits were divided into three equal groups, 18 animals each. All groups were subjected to intra-peritoneal [IP] injection of 20 ml normal saline for group I [control], 20 ml of 50% glucose [G] for group II, and 2g/kg of 50% ethanol [E] [weight/volume] for group III. Sixty minutes after injection, all animals were sacrificed and left to putrefy for 24 ours at room temperature. Blood [B], vitreous humor [V] and urine [U] samples ere collected from each animal at 3 time intervals; I-Before commencing the experiment [0 time]: a blood sample was collected to determine blood glucose concentration [BGC-0] and Blood Ethanol Concentration [BEC-0]. 2-Sixty minutes after injection [just after animal sacrifice]: blood and vitreous humor samples to determine [BGC-60], [BEC-60] and [VEC-60] 3-Twenty-four hours postmortem blood, vitreous humor and urine samples to determine [BEC-24]. [VEC-24] and [UEC-24]. The results showed that there is a postmortem production of ethanol. In group I, the concentration of blood ethanol produced PM ranged from 0-42 mg/dL. In group II [glucose dosed group], this concentration ranged from 7-90 mg/dL. In group three [ethanol dosed group] BEC-24 was significantly higher than BEC-60. On the other hand, a positive correlation was found between BECF-60 and VEC-60, BEC-60 and VEC-24 and BEC-60 and UEC-24 in the ethanol dosed group. Furthermore, in the ethanol dosed group also, the ratio between UEC-24; BEC-60 was 1.236: 1, UEC-24: BEC-24 was 1.212: 1, UEC-24: VEC-24 was 1.219: I and UEC-24; VEC-60 was 1.161:1

RESUMO

Examination of the penile artery by duplex scanning in the assessement of impotence was evaluated in 140 impotent men. Scanning was by Technicare, Diasonic DRF400, and penile artery measurements were taken before and after intra-corporeal injection of papaverin hydrochloride 60 mg. The penile brachial index was measured in 80 patients and its predictive value compared with the resuls of duplex scanning and papaverine induced erection. On scanning evidence of good arterial in flow but poor erections, indirect evidence of venous leakage was assumed. The results showed that the deep artery responses best characterised the erectile response, with the dorsal artery being less helpful. All 30 patients with full erections following papavrine exhibited bilateral deep artery peak Velocities of > 25 cm/s, of the remaining 110 sub-optimal responders, 20 also has this finding; all had undergone dynamic cavernosograpliy, with 19 exhibited venous leakae. A critical value of deep artery response to attain erection is postulated, enabling more logical use of cavernosogrpahy. The penile/brachial index was shown to be suspected and it was concluded that duplex scanning is a useful, non-invasive method in the assessment of imptence

RESUMO

This study was designed to evaluate post operative and long term follow up wound healing after using two methods of repair [direct closure or covering by split thickness skin grafts. Long term follow up revealed a higher incidence of chronic skin ulceration the split-thickness skin graft group as compared with the primary closure group. All skin breakdowns appeared in the presence of C.S.F. leakage and or the presence of Gibbus deformity. A thoracic or thoracolumbar myelomeningocele repair with split thickness skin graft was more likely to be complicated by skin problems than the defects in the lumbar lumbosacral or sacral region. This relationship was secondary to the frequency of gibbus deformity in the more cephalad defects than caudal defects
